Hospital discharge – existing services

There are a variety of professionals at the hospital involved in helping people home. The Integrated discharge team deals with more complex cases, while Hospital at Home manages follow-up tests and medications. Occupational therpists and physiotherapists. the Rapid Response service and the local authority’s reablement service, which social workers can putin place for short-term support while individuals are waiting for assessment, all play a part, too.

Smoothing the way home

Our Hospital Discharge Care Connector Service, led by Jo Foster, complements existing discharge services, addressing practical issues that may be preventing discharge for patients who are otherwise ready to go home. It’s based on-site at the hospital and its goal is to help patients likely with a 0-1 graded discharge pathway who need some extra assistance getting things ready to avoid delays.

Typical support we can supply includes:

Help to arrange handyman services and essential repairs
Organising moving furniture to make the home ready, particularly if there’s a need for downstairs living
Getting hold of house keys, arranging for locks repairs, installing key safes
Social prescribing: connecting patients with emotional wellbeing support and help to access local services longer-term
Energy services: topping up gas and electric meters

The Hospital Discharge Care Connector service is funded by Cheshire West & Chester Council Adult Health and brings third-sector support into the hospital.
We’ve been busy building links with hospital staff and within the first six months of setting up the service we were able to help 78 patients.
