Hospital discharge support at the Countess of Chester Hospital and Leighton Hospital in Crewe
The Community Home First (CFH) hospital discharge service fis led by Cheshire Community Action. It supports medically fit people out of hospital and back home, then assists with arranging comprehensive support to prevent re-admission. CFH is based on-site at the Countess of Chester Hospital and Leighton Hospital in Crewe, and its goal is to help patients with a 0-1 graded (lower level) discharge pathways who need some extra assistance getting things ready to avoid delays. Our Community Connectors complement existing discharge services by addressing practical issues preventing hospital discharge; they can also advise on getting support from other organisations once patients are home.
Help after discharge
After discharge, local social enterprise Snow Angels offers six weeks of support at home for older patients identified with the help of clinical staff. This support includes check-ins, shopping and prescription drop-offs.
Scheme partner Age UK Cheshire also offers dedicated home visits to newly discharged patients who require more holistic and comprehensive support to support their recovery and independence.
Cheshire and Warrington Carers Trust completes the service by providing specialist support at a vital time to carers who have been in hospital themselves and have just got home.
