The Hospital Community Pathfinder is a pivotal role within integrated discharge at the Countess of Chester and Ellesmere Port Hospitals.

Job Title: Community Pathfinder – Hospital Discharge (Part-Time)
Organisation: Cheshire Community Action
Location: Hybrid – Countess of Chester Hospital (COCH), Ellesmere Port Hospital (EPH), and some home working
Salary: £31,022 pro-rata
Hours: 30 hours per week (Tuesday to Friday)

Responsible for enhancing the hospital discharge process by facilitating smooth transitions for patients from hospital care to their homes through coordination with the wider voluntary sector and other partners. This role aims to reduce unnecessary delays in hospital care and improve patient outcomes by creatively seeking alternative community-based provisions through social prescription and utilising 3rd sector knowledge.
