Role at a glance

Job title: Community Pathfinder – Hospital Discharge

Hours: 30 hours per week, Tuesday to Friday

Salary: £32,046 pro rata

Location: Hybrid working across the Countess of Chester Hospital, Ellesmere Port Hospital and home

About the role

When someone is medically ready to leave hospital, everyday challenges can sometimes delay their return home. They may have no one nearby to check on them, be worried about money, or simply be unsure what support is available.

As a Community Pathfinder, you will be based on hospital wards, speaking with patients and their families to understand what would help. Working alongside hospital discharge teams, you will connect people with local community groups and services that can support them once they are home.

Who we are looking for

You do not need a health or social care background. What matters is that you are comfortable talking with people, listen well and enjoy finding practical solutions. We will give you the training and support you need to succeed in the role.
