Our new Hospital Community Care Connector service helps patients have a smooth transition from hospital care to their homes, and coordinates ongoing support to help with their recovery. The success of the service, which started in January 2023, means we are expanding the team.

The Hospital Community Care Connector is a pivotal role within integrated discharge at the Countess of Chester Hospital, responsible for enhancing the hospital discharge process by facilitating smooth transitions for patients from hospital care to their homes through coordination with the wider voluntary sector and other partners. This role aims to reduce unnecessary delays in hospital care and improve patient outcomes by creatively seeking alternative community-based provisions through social prescription and utilising 3rd sector knowledge.

We are looking at creating additional posts of this type as part of a project expansion to increase capacity and cover mid-Cheshire and out-of-area patients.
